Job Description

Advance your nursing career with an engaging field-based role providing essential care in patient homes. This full-time, 13-week assignment offers the opportunity to work Monday through Friday, 8:00 AM 5:00 PM, managing a caseload of approximately 30 points per weektypically 35 patient visits each day.

Schedule & Setting:

  • Monday Friday, 8:00 AM 5:00 PM
  • Full-time, 13-week contract
  • Field-based: daily travel to patient homes (mileage reimbursement provided)

Responsibilities:

  • Perform comprehensive OASIS assessments and develop individualized care plans.
  • Deliver skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management.
  • Monitor patient progress, note changes in condition, and communicate findings promptly with the interdisciplinary care team.
  • Educate patients and families about health conditions, medications, and ongoing care needs.
  • Accurately document all interventions and assessments in Home Care Home Base (HCHB).
  • Partner with other healthcare professionalsphysical and occupational therapists, social workers, and home health aidesto coordinate care.
  • Participate in case conferences, agency meetings, and relevant continuing education.
  • Adhere to regulatory standards and agency policies for patient care and documentation.

Qualifications:

  • Valid RN license in good standing for the state of practice.
  • At least 1 year of recent Home Health experience (field expertise highly valued).
  • Demonstrated proficiency using Home Care Home Base (HCHB) and OASIS assessments.
  • Reliable transportation, active drivers license, and auto insurance.

$1,742 - $1,843/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
