Step into a rewarding nursing career as a Registered Nurse specializing in Home Health care through a 13-week, full-time contract. This field-based opportunity is designed for dedicated RNs who thrive in an autonomous, patient-focused environment, conducting visits and delivering care directly in patient homes.

Schedule & Hours:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 30 points per week (generally 35 home visits per day)

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent, hands-on Home Health nursing experience
• Proficiency with Home Care Home Base (HCHB) electronic documentation
• OASIS assessment experience
• Valid drivers license, reliable transportation, and current auto insurance

Primary Responsibilities:

• Complete thorough OASIS assessments and formulate individualized patient care plans
• Provide skilled nursing services including wound care, medication administration, IV therapy, and chronic disease management
• Continuously evaluate patient status, identify changes, and coordinate with the interdisciplinary care team and physicians
• Deliver crucial patient and family education regarding treatment, medication, and preventive health measures
• Maintain meticulous, timely documentation using HCHB
• Collaborate with therapists, home health aides, and medical social workers for coordinated care
• Participate in case conferences and ongoing professional development
• Uphold all regulatory and agency standards for quality and documentation

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
