Schedule:
- Full-time, Monday through Friday
- Potential on-call responsibilities
- Daily in-home patient visits
What youll do:
- Deliver skilled nursing care in home settings, focusing on quality and patient comfort
- Complete thorough patient assessments, ensuring timely and accurate OASIS documentation
- Collaborate with physicians and interdisciplinary team members to create individualized care plans
- Perform wound care, disease monitoring, medication management, and patient/caregiver education
- Communicate effectively regarding changes in patient condition to ensure optimal care
Qualifications:
- Active RN license
- At least 1 year of recent home health RN experience
- Experience with OASIS documentation (required)
- Familiarity with HomeCare HomeBase (strongly preferred)
- Reliable transportation and a valid drivers license
- Ability to manage a structured daily schedule and routine travel
