Key Qualifications:

• Current and unencumbered Registered Nurse license
• Minimum of one year of recent experience in home health or acute care settings
• Proficiency in patient assessments, treatment planning, and electronic documentation
• Strong critical thinking, decision-making, and problem-solving abilities
• Compassionate communicator, with the ability to interact effectively with patients, families, and the healthcare team
• BLS certification required

Schedule:

• Flexible schedule based on patient assignments and caseload

Primary Responsibilities:

• Assess, plan, implement, and evaluate nursing care for home health patients
• Coordinate and deliver individualized care plans, including medication administration and monitoring vital signs
• Provide patient and family education to promote self-management and safe home environments
• Communicate efficiently with physicians, multidisciplinary teams, and care coordinators to enhance patient outcomes
• Document all care provided, observations, and changes in patient status according to policy
