Deliver vital patient care directly in the comfort of their homes as a Home Health Registered Nurse. This full-time, 13-week contract position offers the opportunity to make a meaningful impact in a field-based setting, providing one-on-one care and support to individuals throughout their recovery process.

Schedule & Setting:

• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 30 points per week (typically 35 patient visits per day)
• Field-based role with daily travel within an assigned territory (mileage reimbursement included)

Key Duties:

• Perform thorough OASIS assessments and develop individualized care plans tailored to each patient's needs.
• Deliver skilled nursing interventions, including wound care, IV therapy, medication management, and chronic disease support.
• Monitor patient conditions, identify changes, and communicate promptly with the care team and physicians.
• Educate patients and families about diagnoses, treatments, medications, and preventive strategies.
• Ensure complete, accurate documentation in Home Care Home Base (HCHB).
• Collaborate with therapists, social workers, and home health aides for a seamless care experience.
• Participate in team meetings, case conferences, and ongoing education.
• Uphold all regulatory and agency care standards.

Qualifications:

• Active RN license (in good standing) for the state of practice
• At least 1 year of recent home health experience (field setting preferred)
• Proficient in Home Care Home Base (HCHB) and OASIS assessments
• Reliable transportation, valid drivers license, and current auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
