Make a meaningful impact on patients lives by delivering high-quality care in their homes as a Registered Nurse in a field-based home health setting. This is a full-time, 13-week contract with a consistent Monday Friday schedule (8:00 AM 5:00 PM), located in the Albuquerque, NM area. Expect to manage a caseload of approximately 35 home visits per day (30 points per week).

Key Responsibilities:

• Complete comprehensive OASIS assessments and develop individualized care plans tailored to patient needs.
• Deliver skilled nursing care, including wound care, medication administration, IV therapy, and chronic disease management in patient homes.
• Monitor patient progress, recognize condition changes, and communicate promptly with interdisciplinary team members and physicians.
• Provide education to patients and families regarding treatment plans, medications, and disease management.
• Maintain timely and accurate documentation using Home Care Home Base (HCHB).
• Collaborate with an interdisciplinary teamincluding therapists, social workers, and aidesfor seamless, coordinated care.
• Attend case conferences, staff meetings, and participate in ongoing education.
• Uphold regulatory and agency standards for patient care and documentation compliance.

Preferred Qualifications:

• Active RN license (in good standing) in the state of practice.
• At least one year of recent home health experience (field experience preferred).
• Proficiency with Home Care Home Base (HCHB) and OASIS required.
• Reliable transportation, valid drivers license, and auto insurance.

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
