Take your nursing expertise beyond the walls of a facility and into patients homes with this full-time, 13-week contract as a registered nurse specializing in home health care. You will be field-based, providing personalized care and support to individuals within their residences, making a meaningful impact each day.

Schedule & Hours:

• Monday to Friday, 8:00 AM 5:00 PM

Your Responsibilities:

• Complete comprehensive patient assessments using OASIS and develop individualized care plans.
• Deliver skilled nursing care such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes, and communicate updates promptly to physicians and the interdisciplinary care team.
• Provide patient and family education on disease processes, treatment plans, medications, and preventive healthcare.
• Utilize the Home Care Home Base (HCHB) system to document all care and interventions accurately and in a timely manner.
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides for optimal patient outcomes.
• Participate in case conferences, staff meetings, and ongoing education as required.
• Ensure compliance with regulatory standards and agency guidelines for home health nursing.

Qualifications:

• Current, active RN license (state-specific, in good standing)
• At least 1 year of recent home health experience (fieldwork strongly preferred)
• Proficient with Home Care Home Base (HCHB)
• Demonstrated experience using the OASIS assessment tool
• Reliable transportation, valid drivers license, and maintained auto insurance

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
