Job Description

Deliver high-quality nursing care in a field-based home health setting, supporting patients in their homes and ensuring exceptional clinical outcomes. This full-time 13-week assignment offers a Monday through Friday, 8:00 AM5:00 PM schedule, with an expected caseload of approximately 35 patient visits per day (totaling around 30 points per week).

Desired Qualifications:

• Active RN license in the state of practice and in good standing
• At least one year of recent experience in home health, with a preference for prior field-based work
• Proficient in Home Care Home Base (HCHB) electronic documentation
• Demonstrated experience with OASIS assessments and related care planning
• Valid drivers license, current auto insurance, and reliable transportation for daily travel

Primary Responsibilities:

• Perform thorough OASIS assessments and establish individualized care plans for patients
• Deliver skilled nursing care, including IV therapy, medication administration, wound care, and management of chronic conditions
• Evaluate patient status, communicate changes promptly with care team and medical providers, and adapt care as needed
• Provide education to patients and families on disease management, medications, and health maintenance
• Maintain accurate clinical documentation using Home Care Home Base (HCHB)
• Coordinate care with therapists, medical social workers, and home health aides to deliver integrated services
• Attend case conferences, participate in staff meetings, and pursue continuing education opportunities
• Ensure compliance with agency standards and regulatory requirements in all care activities

$2,057 - $2,176/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
