Job Description
Seeking an experienced clinical leader to oversee and coordinate nursing and patient care activities in a dynamic home health environment. This contract position requires a strong nurse manager or clinical nurse specialist (CNS) with demonstrated skills in managing interdisciplinary teams, ensuring compliance, and delivering exceptional care.
Responsibilities include:
- Individualizing patient care plans, ensuring they are current and reassessed with significant health changes.
- Overseeing documentationreviewing evaluation packets, OASIS, and 485 forms; ensuring all documentation is accurate and timely.
- Processing OASIS/485 information, verifying correct start of care and episode dates.
- Managing orders: review, approval, corrections, and ensuring compliance for medication and supplies.
- Leading weekly case conferences, making recertification/discharge decisions, and documenting accordingly.
- Serving as a resource for referral intake and ensuring timely assessment visits.
- Contacting physicians for ongoing or additional orders as needed.
- Reviewing administrative issues, including entitlement verification and billing claims.
- Functioning as a clinical back-up, participating in on-call rotation and PRN patient visits as needed.
- Acting as backup for patient services coordination and workflow support.
Desired qualifications and experience:
- Graduate of an accredited professional nursing program with an active RN license in the state of employment.
- Minimum of two years nursing experience, with at least one year in home health.
- Supervisory or management experience in a clinical setting is highly preferred.
- Strong communication abilities, both written and oral; excellent organizational skills.
- Proficient in OASIS and HomeCare HomeBase.
- Solid understanding of regulatory guidelines for home health agencies.
- Valid drivers license and automobile liability insurance; must be able to drive in all weather conditions.
