A compassionate Licensed Practical Nurse (LPN) is needed to provide high-quality home health care to patients within a designated territory. This contract-based, full-time opportunity empowers you to deliver individualized nursing care while supporting patient independence and recovery in a home setting. The position values strong clinical judgment, autonomy, and excellent communication with patients, their families, and the healthcare team.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM (occasional flexibility based on patient needs)
• Field-based, covering home visits within assigned territory
• Manage 2532 points per week (equivalent to approximately 35 patient visits per day)

Qualifications:

• Active and unrestricted LPN license in the state of practice
• Minimum of 1 year of prior experience in home health, hospice, or related community care
• Proficient with Home Care Home Base (HCHB) software (required)
• Current BLS (Basic Life Support) certification
• Valid drivers license, reliable vehicle, and current auto insurance
• Excellent clinical assessment, documentation, and communication skills
• Ability to work independently and manage a dynamic caseload

Preferred:

• Experience in Medicare-certified home health agencies
• Strong understanding of home health compliance and documentation standards
• Patient-centered, empathetic approach to care

Key Responsibilities:

• Conduct skilled nursing visits, assess patient needs, and implement individualized care plans
• Administer medications, wound care, IV therapy, and other clinical interventions as ordered
• Educate patients and caregivers in disease management and safe practices
• Accurately document all care and communication in HCHB
• Collaborate with the interdisciplinary team, participate in quality activities, and maintain infection control standards
