Job Description
Make a meaningful impact by delivering high-quality care directly to patients in their homes as a skilled and compassionate Licensed Practical Nurse (LPN). This contract position offers the opportunity to focus on individualized patient care, promote recovery, and foster independence in a home health setting.
Schedule & Role Details:
- Monday Friday, 8:00 AM 5:00 PM, with occasional flexibility for patient needs
- Field-based roleserving patients in their homes within an assigned territory
- Manage a caseload of 2532 points per week (approximately 35 patient visits per day)
- Home Care Home Base (HCHB) charting required
- Reliable transportation is a must; mileage is typically reimbursed
Desired Experience & Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of recent home health, hospice, or community-based nursing experience
- Comfortable and proficient using Home Care Home Base (HCHB)
- Valid drivers license and insured vehicle
- Current BLS certification
- Ability to work autonomously, make sound clinical judgments, and handle a dynamic caseload
- Excellent communication skills and a strong commitment to patient-centered care
- Preferred: Previous experience with Medicare-certified home health agencies and solid knowledge of compliance/documentation standards
Key Responsibilities:
- Conduct skilled nursing visits to assess, plan, and implement individualized care plans
- Administer medications, wound care, IV therapy, and other clinical interventions as ordered
- Monitor patients condition changes and communicate findings to the care team
- Educate patients and caregivers on medications, self-care, and disease management
- Document all assessments and interventions using HCHB
- Uphold infection control standards and participate in ongoing training and quality improvement
