Job Description

Experienced and compassionate nursing professionals are invited to apply for a full-time contract opportunity as a Home Health Licensed Practical Nurse (LPN). In this vital position, you will deliver high-quality, patient-centered care to individuals in their homes, fostering greater independence, education, and recovery. Your expertise will be key in maintaining strong communication channels with patients, families, and the broader healthcare team.

Schedule:

  • Monday through Friday, 8:00 AM 5:00 PM
  • Occasional flexibility required for patient needs

Responsibilities include:

  • Managing a caseload of 2532 points per week (about 35 patient visits per day) within an assigned territory
  • Conducting skilled nursing visits and creating individualized care plans
  • Providing medication administration, wound care, and IV therapy as prescribed
  • Monitoring patient conditions and reporting changes to the RN Case Manager or physician
  • Delivering education on disease processes, medication routines, and self-care practices to patients and caregivers
  • Complete, timely documentation of care using Home Care Home Base (HCHB)
  • Coordinating with an interdisciplinary team and ensuring compliance with all care standards
  • Participating in case conferences, training, and quality improvement initiatives
  • Upholding infection control standards and ensuring a safe environment for all patients

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum one year of home health, hospice, or similar experience
  • Proficient with Home Care Home Base (HCHB) charting system
  • Valid drivers license, reliable vehicle, and current insurance
  • Current BLS certification
  • Excellent time management, clinical judgment, and communication abilities
  • Ability to work independently and manage a dynamic, field-based caseload

Preferred:

  • Home health compliance experience, especially with Medicare-certified agencies
  • Strong documentation skills and a patient-first approach
