Job Description

Make a meaningful impact by delivering compassionate, high-quality care in a home health setting. This contract opportunity seeks an experienced Licensed Practical Nurse (LPN) with a strong background in community-based nursing to serve patients within their homes and help promote independence, recovery, and education.

Schedule/Hours:

  • Monday Friday, 8:00 AM 5:00 PM
  • Occasional flexibility for patient needs

Role Overview:

  • Field-based position covering an assigned territory
  • Manage a caseload of 2532 points per week (about 35 patient visits per day)
  • Travel required; reliable transportation and mileage reimbursement typically provided
  • Documentation using Home Care Home Base (HCHB) system

Core Responsibilities:

  • Perform skilled nursing visits, assessments, and implement tailored care plans for home health patients
  • Administer medications, wound care, IV therapy, and other clinical interventions per provider orders
  • Closely monitor patient condition, reporting significant changes to the RN Case Manager or physician
  • Educate patients and caregivers on disease management, medications, and self-care techniques
  • Maintain all charting and communication with Home Care Home Base (HCHB)
  • Ensure infection control, patient safety, and compliance with all care plans and standards
  • Work collaboratively with the interdisciplinary team and participate in case conferences or quality initiatives

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • At least one year of home health, hospice, or similar experience required
  • Proficiency with Home Care Home Base (HCHB)
  • Current BLS certification
  • Valid drivers license, reliable insured vehicle
  • Strong clinical judgment, communication, and time management skills
  • Ability to work independently in a field-based role

Preferred:

  • Prior experience with Medicare-certified home health agencies
  • Knowledge of home health compliance and documentation
