Job Description
An opportunity is available for a skilled and compassionate Licensed Practical Nurse (LPN) with home health experience to deliver quality patient care in a field-based setting. The position focuses on supporting patients within their homes, promoting self-management, and ensuring comprehensive, individualized care. Your clinical expertise and patient-centered approach will help patients achieve maximum health and independence.
Schedule:
- Monday Friday, 8:00 AM 5:00 PM
- Occasional flexibility to meet patient needs
Key Responsibilities:
- Provide skilled nursing visits and develop individualized care plans for home health patients
- Administer medications, perform wound care, and manage IV therapy or other clinical procedures
- Monitor patients conditions and communicate changes to the RN Case Manager or physician
- Educate patients and caregivers about illness, medications, and care techniques
- Thoroughly document assessments, care, and communication using Home Care Home Base (HCHB)
- Collaborate with the interdisciplinary healthcare team to coordinate patient care
- Maintain infection control and ensure patient safety at all times
- Participate in case conferences, training, and quality improvement activities
Qualifications:
- Active, unrestricted LPN license in the state of practice
- At least 1 year of experience in home health, hospice, or community-based care
- Proficient in Home Care Home Base (HCHB) charting system
- Valid drivers license, reliable transportation, and current insurance
- Current BLS certification
- Strong communication, time management, and independent decision-making skills
Preferred Experience:
- Work with Medicare-certified home health agencies
- Familiarity with compliance and documentation standards in home health
- Excellent interpersonal skills and dedication to patient care
