Job Description

Provide essential nursing care in a dynamic skilled nursing environment focused on short-term and long-term patient recovery. The setting specializes in caring for individuals overcoming illnesses, injuries, or accidents with conditions such as stroke, amputation, neurological disorders, and post-joint replacement. This contract role calls for a Licensed Practical Nurse well-versed in long-term care with a minimum of 18 months relevant experience in SNF/LTC rehab settings.

Key Responsibilities:

  • Deliver high-quality LPN care with a patient ratio of approximately 1:18-20 on days and 1:26 at night.
  • Manage tracheostomies, Foley catheters, wound care, dressing changes, IV starts/management, and tube feedings.
  • Collaborate with nursing leadership, case management, therapies, and support teams.
  • Use EPIC EMR system, Alaris IV pumps, and Omnicell medication dispensing technology.
  • Float within continuing care facilities according to regional needs while remaining within scope of practice.
  • Participate in a 2-day orientation covering departmental and facility protocols.
  • Work 12-hour shifts, averaging 36 hours weekly, with weekend rotation every other weekno call requirement but holiday coverage expected at 75%.

Qualifications:

  • Valid LPN license with current BLS certification.
  • At least 18 months experience in long-term care or rehab settings preferred.
  • Skilled in trach management, Foley care, wound treatment, IV therapy, and tube feeding.
  • Comfortable with floating to several regional facilities as needed.
  • Ability to adapt to busy clinical settings with diverse patient populations.

Additional Information:
