Job Description

Deliver high-quality, compassionate nursing care where it matters mostin your patients homes. Skilled Licensed Practical Nurses are sought for a full-time, contract role providing individualized care as part of a dedicated home health team.

Schedule:

  • MondayFriday, 8:00 AM5:00 PM
  • Occasional flexibility as required for patient needs

In this field-based position, you will manage a caseload of 2532 points per week (35 patient visits daily) within an assigned territory, focusing on patient education, promoting independence, and supporting recovery in the comfort of patients homes. Proficiency with the Home Care Home Base (HCHB) charting system is essential.

Core Responsibilities:

  • Conduct skilled nursing assessments and deliver individualized patient care
  • Administer medications, wound care, IV therapy, and other interventions per provider orders
  • Monitor and report patient status changes promptly to the RN Case Manager or physician
  • Teach patients and families about disease management, medications, and self-care strategies
  • Accurately document visits and care provided in the HCHB platform
  • Collaborate with the interdisciplinary healthcare team to coordinate comprehensive care
  • Maintain infection control and safety standards at all times
  • Participate in case conferences, staff meetings, and any required professional training

Qualifications:

  • Active, unrestricted LPN license in the state of practice
  • Minimum one year of home health, hospice, or related community-based experience
  • Mandated proficiency in Home Care Home Base (HCHB) documentation
  • Current BLS certification, valid drivers license, and reliable transportation
  • Strong clinical judgment and time management abilities
  • Effective independent work capabilities and communication skills
  • Preferred: Experience with Medicare-certified home health agencies and familiarity with compliance standards
