Providing compassionate, patient-focused nursing care in home settings, this full-time contract opportunity is tailored for experienced Licensed Practical Nurses (LPNs) skilled in home health nursing. Based in Hendersonville, TN, the role involves delivering high-quality skilled nursing services to patients within their homes, supporting independence and recovery through effective care and education.
Responsibilities:
- Perform skilled nursing assessments and develop individualized care plans for home health patients.
- Administer medications, wound care, IV therapy, and other clinical interventions as prescribed.
- Monitor patient condition changes and communicate promptly with RNs or physicians.
- Educate patients and caregivers on disease management, medication adherence, and self-care techniques.
- Document all nursing assessments and care provided accurately using Home Care Home Base (HCHB) software.
- Collaborate with interdisciplinary healthcare teams to ensure coordination and compliance with care plans.
- Adhere to infection control protocols and maintain patient safety during all visits.
- Participate in case conferences, quality improvement initiatives, and ongoing training.
Qualifications/Desired Experience:
- Active, unrestricted LPN license in the state of Tennessee.
- Minimum one year of experience in home health, hospice, or related community-based care.
- Proficiency with Home Care Home Base (HCHB) charting system.
- Valid drivers license, reliable vehicle, and current insurance.
- Current Basic Life Support (BLS) certification.
- Excellent clinical judgment, time management, and communication skills.
- Ability to work independently managing a dynamic caseload in the field.
- Preferred: Experience with Medicare-certified home health agencies and strong knowledge of home health compliance standards.
