Job Description

Make a difference in patients lives as a dedicated Home Health Licensed Practical Nurse (LPN). This field-based, contract opportunity is ideal for clinically experienced nurses who thrive in a dynamic, independent environment and are passionate about supporting patients in the comfort of their own homes.

Schedule:

  • Monday to Friday, 8:00 AM 5:00 PM
  • Occasional schedule flexibility to meet patient needs

Key Responsibilities:

  • Conduct skilled home visits to assess, plan, and deliver individualized patient care
  • Implement care plans, administer medications, perform wound care, manage IV therapies, and other clinical interventions
  • Monitor patient conditions; promptly communicate changes to the RN Case Manager or physician
  • Provide patient and caregiver education on disease processes, medications, and self-care techniques
  • Document all care and communications accurately using Home Care Home Base (HCHB)
  • Coordinate care with the broader interdisciplinary team and participate in case conferences
  • Ensure compliance with regulatory standards, maintain infection control, and prioritize patient safety at all times
  • Participate in quality improvement activities and required ongoing training

Required Qualifications:

  • Active and unrestricted LPN license
  • Minimum of 1 years experience in home health, hospice, or related community-based care
  • Experience with Home Care Home Base (HCHB) charting system
  • Valid drivers license and reliable transportation, with up-to-date insurance
  • Current BLS certification
  • Ability to independently manage a field-based caseload and document care accurately
  • Excellent clinical judgment, time management, and communication skills

Preferred:

  • Prior work for Medicare-certified home health agencies
  • Strong knowledge of home health compliance and documentation standards
