Qualifications and Experience:

• Current, active LPN license in good standing
• Experience providing patient-centered care in a healthcare facility
• Knowledge of nursing best practices, medication administration, and infection control procedures
• Strong clinical assessment and patient documentation skills
• Ability to work collaboratively within multidisciplinary teams
• Compassionate, attentive, and professional communication abilities
• CPR/BLS certification preferred

Key Responsibilities:

• Monitor vital signs, report changes in patient conditions, and assist with ongoing evaluations
• Support daily living activities, such as bathing, dressing, and mobility assistance
• Document patient status, interventions, and responses in a timely and accurate manner
• Communicate effectively with patients, families, and the wider medical team
• Maintain a clean, safe, and compliant care environment
• Follow all regulatory guidelines, protocols, and policies
