Job Description

Bring your nursing expertise to a meaningful home health setting, providing compassionate and high-quality care to patients in their own residences. This contract role emphasizes patient independence, caregiver education, and collaboration across the care team. As a Home Health Licensed Practical Nurse (LPN), you will manage a field-based caseload, contributing to positive patient outcomes and maintaining compliance with regulatory guidelines.

Schedule & Work Details:

  • Monday through Friday, 8:00 AM 5:00 PM, with some flexibility based on patient needs
  • Full-time contract assignment
  • Manage a workload of approximately 2532 points per week (about 35 patient visits daily)
  • Travel within an assigned territory, mileage reimbursement typically provided

Key Responsibilities:

  • Conduct skilled nursing visits to assess, plan, and implement tailored care plans for patients at home
  • Administer medications, perform wound care, IV therapy, and clinical interventions as prescribed
  • Monitor patient status, documenting findings and promptly communicating any changes to the case manager or physician
  • Educate patients and caregivers on diagnosis, medication regimens, and self-care techniques
  • Ensure accurate documentation of all care and communication using Home Care Home Base (HCHB)
  • Collaborate with the interdisciplinary team to coordinate comprehensive care
  • Adhere to infection control practices and uphold patient safety standards
  • Participate in team meetings, quality improvement initiatives, and ongoing training

Desired Qualifications:

  • Active, unrestricted LPN license in the state
  • At least 1 year of experience in home health, hospice, or a comparable field-based care setting
  • Proficiency in Home Care Home Base (HCHB) charting
  • Current BLS certification; valid driver's license and insured, reliable vehicle
  • Strong assessment, organizational, and communication skills

Preferred:

  • Experience with Medicare-certified agencies
  • Strong grasp of home health compliance and documentation
