Job Description

Advance your practice as a Nurse Practitioner specializing in home health risk assessments with a dynamic per diem opportunity. This role focuses on providing meaningful clinical evaluations for Medicare Advantage plan members throughout Scioto County. Your nursing expertise will contribute to enhanced patient outcomes and help bridge gaps in care, all while enjoying flexible scheduling designed around your availability.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling create a work itinerary that fits your lifestyle
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members, with each assessment averaging 45 minutes
  • Travel to member residences throughout Scioto County, including Portsmouth, Porter Township, and Clay Township
  • Review and document prior/current medical history, existing conditions, and medication regimen
  • Educate individuals about their health conditions and medications to encourage better self-management
  • Identify and address gaps in care, offering recommendations for improved health outcomes
  • No prescribing or creation of treatment plans required in this role

Qualifications:

  • Valid Nurse Practitioner License in Ohio
  • Board Certification (AANP or ANCC)
  • MSN from an accredited nursing program
  • Current CPR certification (BLS)
  • At least 6 months of recent NP experience preferred
