Advance your practice as a Nurse Practitioner specializing in home health risk assessments with a dynamic per diem opportunity. This role focuses on providing meaningful clinical evaluations for Medicare Advantage plan members throughout Scioto County. Schedule:

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members, with each assessment averaging 45 minutes
• Travel to member residences throughout Scioto County, including Portsmouth, Porter Township, and Clay Township
• Review and document prior/current medical history, existing conditions, and medication regimen
• Educate individuals about their health conditions and medications to encourage better self-management
• Identify and address gaps in care, offering recommendations for improved health outcomes
• No prescribing or creation of treatment plans required in this role

Qualifications:

• Valid Nurse Practitioner License in Ohio
• Board Certification (AANP or ANCC)
• MSN from an accredited nursing program
• Current CPR certification (BLS)
• At least 6 months of recent NP experience preferred
