Deliver compassionate, patient-focused care as a Nurse Practitioner specializing in home health risk assessments on a per diem basis. This flexible opportunity enables you to create your own schedule with shifts available between Monday and Sunday, and requires a minimum weekly or bi-weekly hourly commitment based on your availability. Assignments will take place within Columbiana County, including Perry Township, Salem, and East Liverpool, where youll be scheduled depending on patient needs for each shift.

Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members
- Assess patient history, review current and past medical conditions, and evaluate medication regimens
- Offer education to patients regarding their medical conditions and prescribed medications
- Identify gaps in healthcare and recommend solutions
- No requirement for developing treatment plans or prescribing medication
Qualifications:
- Active Nurse Practitioner License in Ohio
- Board Certification via AANP or ANCC
- Current BLS/CPR certification
- MSN from an accredited nursing school
- Minimum of 6 months experience as a Nurse Practitioner preferred
