Provide expert home health risk assessments as a Nurse Practitioner, supporting Medicare Advantage plan members in Columbiana County, OH. This per diem position offers flexible scheduling options, allowing you to create a personalized work schedule between Monday and Sunday with a minimum weekly or bi-weekly hourly commitment.
Key Responsibilities:
- Perform thorough annual home health risk assessments, approximately 45 minutes each, including post-discharge visits
- Evaluate patients medical history, current conditions, and medication regimens
- Educate members on their medical conditions and medications to promote understanding and compliance
- Identify gaps in care and offer actionable recommendations for resolution
- Note: No treatment plan creation or medication prescribing is required
Qualifications:
- Valid Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months of Nurse Practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Schedule:
- Flexible shifts available Monday through Sunday
