Job Description

Provide expert home health risk assessments as a Nurse Practitioner, supporting Medicare Advantage plan members in Columbiana County, OH. This per diem position offers flexible scheduling options, allowing you to create a personalized work schedule between Monday and Sunday with a minimum weekly or bi-weekly hourly commitment.

Key Responsibilities:

  • Perform thorough annual home health risk assessments, approximately 45 minutes each, including post-discharge visits
  • Evaluate patients medical history, current conditions, and medication regimens
  • Educate members on their medical conditions and medications to promote understanding and compliance
  • Identify gaps in care and offer actionable recommendations for resolution
  • Note: No treatment plan creation or medication prescribing is required

Qualifications:

  • Valid Nurse Practitioner license in Ohio
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of 6 months of Nurse Practitioner experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution

Schedule:

  • Flexible shifts available Monday through Sunday
  • Ability to design your own schedule to fit your lifestyle
  • Minimum hourly commitment weekly or bi-weekly
