Job Description

Take on a highly rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members in the Westerville, OH region. In this flexible role, you will visit patients residences across Brown Countyincluding Pleasant Township, Georgetown, Mount Orab, and Ripleyto complete thorough evaluations and support proactive, patient-centered care. Each home health risk assessment takes about 45 minutes and may include annual health checkups or post-discharge visits.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Build a schedule that fits your needs
  • Commit to a minimum number of hours weekly or bi-weekly

Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Evaluate both prior and current medical history, document medical conditions, and review medication regimens
  • Educate patients on their health conditions and medications to promote better self-management
  • Identify care gaps and offer actionable recommendations to help close them
  • Please note: This role does not require the creation of treatment plans or prescription of medications

Qualifications:

  • Active Nurse Practitioner licensure in Ohio
  • Current BLS CPR certification
  • Board Certification as NP (AANP or ANCC)
  • Six months of recent nurse practitioner experience is highly preferred
  • Master of Science in Nursing (MSN) from an accredited institution
