A rewarding opportunity is available for a Nurse Practitioner specializing in Home Health to perform essential risk assessments for Medicare Advantage plan members within Adams County, Ohio. This per diem role offers flexible scheduling across locations including West Union, Manchester, and Peebles, allowing you to create a work schedule that suits your lifestyle.

Key Responsibilities:

  • Conduct annual home health risk assessments, focusing on Medicare Advantage members
  • Review prior and current medical histories, conditions, and medication use
  • Educate patients on their medical conditions and medications to promote better health outcomes
  • Identify and address gaps in care by providing informed recommendations
  • Note: This role does not require creating treatment plans or prescribing medications

Qualifications:

  • Valid Nurse Practitioner license in Ohio
  • Current Basic Life Support (BLS) CPR certification
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing school
  • Preferred: Minimum of 6 months recent NP experience

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule with a minimum weekly or bi-weekly commitment
