This per diem opportunity is ideal for a dedicated and compassionate Nurse Practitioner interested in home health care. Youll play a vital role in conducting risk assessments for Medicare Advantage plan members throughout the greater Scioto County area. This position allows you to create your own schedule with flexible shifts, making it perfect for those seeking work-life balance.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own hours and schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members
• Assess prior and current medical history, recorded conditions, and medication history
• Educate patients on their medical conditions and prescribed medications
• Identify care gaps and suggest actionable recommendations for care improvement
• Maintain documentation in accordance with health plan standards
• No treatment planning or medication prescribing necessary

Qualifications:

• Active Nurse Practitioner license in Ohio
• Board Certification (AANP or ANCC)
• Current CPR certification (BLS)
• MSN from an accredited nursing school
• At least six months of Nurse Practitioner experience preferred

This role offers compensation of $80$90 for each completed evaluation, as well as a mileage stipend and reimbursement for administrative time.
