Provide essential home health risk assessments as a Nurse Practitioner serving Medicare Advantage plan members in Darke County, OH. This per diem role offers flexible scheduling throughout Greenville, Versailles, Arcanum, and surrounding areas, allowing you to tailor work hours to your availability between Monday and Sunday.
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments, averaging 45 minutes per visit
- Evaluate medical history, current conditions, and medication use to assess patient health status
- Educate patients on medical conditions and medication management to promote wellness
- Identify care gaps and suggest appropriate recommendations to improve patient outcomes
- Collaborate effectively without the need to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification through the AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing school
- Minimum of six months experience as an NP preferred
Schedule:
- Flexible shifts, Monday through Sunday
- Create your own weekly or bi-weekly schedule
- Minimum hourly commitment required to maintain caseload
