Job Description

A Nurse Practitioner opportunity is available to perform home health risk assessments for Medicare Advantage plan members residing in Guernsey County, OH. This per diem role offers flexible scheduling with shifts available any day of the week, allowing you to create a schedule that fits your lifestyle with a minimum weekly or bi-weekly hourly commitment.

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments focused on Medicare Advantage members
  • Review prior and current medical history, ongoing medical conditions, and medication use
  • Provide patient education related to medical conditions and medication management
  • Identify gaps in care and recommend strategies to improve patient outcomes
  • Note: This role does not require creating treatment plans or prescribing medications

Qualifications:

  • Current Nurse Practitioner License in Ohio
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing school
  • Minimum of 6 months recent NP clinical experience preferred
  • Current CPR certification (BLS)

Location: Assessments will be conducted primarily within Guernsey County, OH, encompassing areas such as Cambridge Township, Cambridge, and Jackson Township. This region offers a suburban and small-town atmosphere with close-knit communities nestled within scenic landscapes, providing a comfortable and accessible working environment for healthcare professionals.

This position offers a per diem schedule with flexible shifts from Monday through Sunday. You will receive compensation per evaluation completed plus a mileage stipend and administrative time, supporting your work in the field.
