Deliver high-quality patient care while enjoying autonomy and flexibility as a Nurse Practitioner specializing in home health services. In this per diem position, you will conduct comprehensive home health risk assessments for Medicare Advantage plan members throughout various communities in Portage County, OH. Locations may include areas such as Kent, Ravenna, Streetsboro, and their surrounding townships.

Schedule:

• Flexible shifts available Monday through Sunday

• Minimum hourly commitment weekly or bi-weekly

Key Responsibilities:

• Perform annual and post-discharge home health risk assessments for Medicare Advantage members in their homes
• Conduct thorough evaluations, reviewing prior/current medical history, conditions, and medication usage
• Educate patients on health conditions and medication regimens
• Identify care gaps and provide actionable recommendations
• Focus solely on assessmentno treatment planning or medication prescribing required

Qualifications:

• Active Nurse Practitioner License in Ohio
• Current CPR certification (BLS)
• Board certified through AANP or ANCC
• Minimum 6 months recent Nurse Practitioner experience preferred
• Masters in Nursing (MSN) from an accredited program

Compensation for this role is provided per evaluation completed, with rates ranging from $80 to $90 and additional mileage stipend and administrative time factored in.
