A Nurse Practitioner role is available to conduct home health risk assessments for Medicare Advantage plan members throughout Hamilton County, OH. This per diem position offers a flexible schedule, allowing you to create your own shifts between Monday and Sunday with a minimum weekly or bi-weekly hourly commitment.
Location Overview:
Hamilton County, OH, encompasses vibrant communities such as Cincinnati, Forest Park, and Norwood. This diverse area combines urban amenities with suburban charm, offering convenient access to cultural attractions, parks, and recreational activities. Assignments may be scheduled anywhere within these locations, providing a dynamic and varied work environment.
Key Responsibilities:
- Perform thorough annual home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical history, existing medical conditions, and medication regimens
- Educate patients on their medical conditions and prescribed medications
- Identify gaps in care and suggest practical recommendations to address them
- Note: This role does not involve creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current Basic Life Support (BLS) certification
- Board certification through AANP or ANCC
- Minimum of 6 months recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited nursing school
Schedule:
- Flexible shifts Monday through Sunday
- Create your own schedule based on availability
- Minimum commitment required weekly or bi-weekly
