Job Description

A Nurse Practitioner role is available to conduct home health risk assessments for Medicare Advantage plan members throughout Hamilton County, OH. This per diem position offers a flexible schedule, allowing you to create your own shifts between Monday and Sunday with a minimum weekly or bi-weekly hourly commitment.

Assignments may be scheduled anywhere within these locations, providing a dynamic and varied work environment.

Key Responsibilities:

  • Perform thorough annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical history, existing medical conditions, and medication regimens
  • Educate patients on their medical conditions and prescribed medications
  • Identify gaps in care and suggest practical recommendations to address them
  • Note: This role does not involve creating treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current Basic Life Support (BLS) certification
  • Board certification through AANP or ANCC
  • Minimum of 6 months recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited nursing school

Schedule:

  • Flexible shifts Monday through Sunday
  • Create your own schedule based on availability
  • Minimum commitment required weekly or bi-weekly
