Job Description

Opportunity for a skilled Nurse Practitioner to provide home health risk assessments to Medicare Advantage members across Fayette County. This per diem role focuses on conducting comprehensive evaluations in locations such as Washington Court House, Jeffersonville, and Bloomingburg, meeting members in the comfort of their homes. Flexible scheduling allows you to design your own shifts between Monday and Sunday, with minimum weekly or bi-weekly hour commitments to fit your lifestyle.

Schedule:

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Review and document prior and current medical history, existing conditions, and all medications
  • Educate members regarding their health conditions and medications
  • Identify gaps in care and provide recommendations to address them
  • Post-discharge and annual health assessment visits included
  • No responsibility for developing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner License in Ohio
  • Current CPR certification (BLS)
  • Board certified via AANP or ANCC
  • Minimum of six months recent experience as a Nurse Practitioner preferred
  • Master of Science in Nursing (MSN) from an accredited institution
