Deliver essential healthcare in a home-based setting as an experienced Nurse Practitioner specializing in Home Health. This per diem opportunity offers flexible scheduling, allowing you to create your own roster with a minimum weekly or bi-weekly commitment. Shifts are available any day between Monday and Sunday, across various locations within Shelby County, including Sidney, Fort Loramie, and Anna.
Key Responsibilities:
- Conduct home health risk assessments for members of a Medicare Advantage health plan
- Complete comprehensive evaluations, including review of prior and current medical history, medical conditions, and medication history
- Offer patient education regarding medical conditions and medication regimens
- Identify care gaps and make recommendations to address them
- Focus on assessment and educationno prescribing of medications or treatment plan development required
Qualifications:
- Active Nurse Practitioner License in Ohio
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months of current NP experience preferred
- Master of Science in Nursing (MSN) from an accredited nursing school
For every evaluation performed, compensation ranges from $80-$90, plus a mileage stipend and administrative time allowanceproviding financial recognition for your expertise and travel.
