Job Description

Seeking a dynamic healthcare professional with Nurse Practitioner credentials to provide essential home health risk assessments for Medicare Advantage plan members. This per diem opportunity allows you to make a meaningful impact by visiting patients throughout various communities, including Sidney, Fort Loramie, and Anna. Schedule:

  • Flexible shifts available between Monday and Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage plan members
  • Review and evaluate prior and current medical history, medication regimens, and overall health status
  • Educate patients on their medical conditions and medication management
  • Identify care gaps and make recommendations to enhance patient outcomes, without prescribing medications or creating new treatment plans

Qualifications:

  • Active Nurse Practitioner License in Ohio
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited institution
  • Minimum of 6 months recent NP experience preferred

This role is ideally suited for a Practitioner seeking a flexible per diem position that prioritizes clinical assessment and patient education, without the demands of ongoing treatment plan management. Compensation for this opportunity ranges from $80 to $90 per completed evaluation, with an added mileage stipend and compensation for administrative time.
