As a Nurse Practitioner, you will play a crucial role in assessing and educating patients directly in their homes, ensuring they receive comprehensive, personalized care and supporting their health management plans.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage plan members
- Review prior and current medical history, medical conditions, and medication lists
- Educate patients about their medical conditions and medications
- Identify any gaps in care and provide expert recommendations for improvement
- Complete comprehensive documentation for each assessment
- No prescribing or treatment plan creation required
Qualifications:
- Current Nurse Practitioner License in Ohio
- Active CPR certification (BLS required)
- Board Certification through AANP or ANCC
- Minimum of 6 months of current NP experience preferred
- MSN obtained from an accredited school of nursing
