Make a meaningful difference in patient care while enjoying flexible per diem work as a Nurse Practitioner providing home health risk assessments. This position allows you to craft your own schedule, supporting Medicare Advantage plan members in areas throughout Darke County, including Greenville, Versailles, and Arcanum. Enjoy autonomy in your practice while engaging with patients in a rewarding home setting.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own working schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual health risk assessments in patient homes for a Medicare Advantage health plan
- Thoroughly evaluate prior and current medical history, assess medical conditions, and review medication lists
- Offer education to patients about their conditions and prescribed medications
- Detect gaps in care and make recommendations for improvement
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Ohio
- Graduate of an accredited Master of Science in Nursing (MSN) program
- Current CPR certification (BLS)
- Board certified by the AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
This per diem opportunity provides highly competitive compensation of $80$90 per evaluation, plus a stipend for mileage and paid administrative time.
