Job Description

Provide comprehensive home health risk assessments for Medicare Advantage plan members across Darke County, OH, including Greenville, Versailles, and Arcanum. As a Nurse Practitioner, you will evaluate patients' medical histories and current conditions, identify gaps in care, and offer education to support improved health outcomes. This per diem role offers flexible scheduling, enabling you to create a work routine that fits your lifestyle with shifts available Monday through Sunday.

Key Responsibilities:

  • Perform annual home health risk assessments on Medicare Advantage members
  • Review prior and current medical and medication histories
  • Educate patients on their medical conditions and medications
  • Identify gaps in care and suggest actionable recommendations
  • No treatment plan creation or medication prescribing required

Qualifications:

  • Active Nurse Practitioner License in Ohio
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing school
  • Minimum six months of recent NP experience preferred

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment
