A rewarding opportunity is available for a Nurse Practitioner specializing in Home Health to perform comprehensive risk assessments within Huron County, OH. This per diem position offers flexible scheduling, allowing you to create your own hours between Monday and Sunday, with a minimum weekly or bi-weekly hourly commitment. This role focuses on conducting home health risk assessments for Medicare Advantage plan members, with visits typically lasting around 45 minutes.

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage enrollees.
  • Evaluate medical history, current conditions, and medication usage.
  • Provide education to patients about their conditions and medications.
  • Identify gaps in care and offer recommendations to address them.
  • Note: This role does not involve prescribing medications or creating treatment plans.

Qualifications:

  • Active Nurse Practitioner license in Ohio.
  • Board Certification through AANP or ANCC.
  • Current CPR/BLS certification.
  • Preferred minimum of 6 months recent NP experience.
  • Master of Science in Nursing (MSN) from an accredited institution.

Schedule:

  • Flexible shifts Monday through Sunday.
