Job Description

Provide critical home health risk assessments for Medicare Advantage members across Erie County, OH, including Sandusky, Huron, and Vermilion. This per diem opportunity offers flexible scheduling, allowing you to create your own hours between Monday and Sunday with a minimum hourly commitment weekly or bi-weekly.

Key Responsibilities:

  • Conduct annual home health risk assessments, including post-discharge visits, averaging 45 minutes per evaluation.
  • Review and evaluate medical history, current medical conditions, and medication use.
  • Educate patients on health conditions and medication management.
  • Identify gaps in care and recommend solutions to improve patient outcomes.
  • Note: This role does not involve creating treatment plans or prescribing medications.

Qualifications:

  • Current Nurse Practitioner license in Ohio.
  • Board Certification through AANP or ANCC.
  • MSN degree from an accredited nursing program.
  • CPR certification (BLS) required.
  • Preferred: Minimum of 6 months recent NP clinical experience.
