Job Description

Take on an impactful role delivering quality healthcare directly to patients homes in your community. As a Nurse Practitioner specializing in home health risk assessments, you will be providing valuable evaluations and support to Medicare Advantage plan members throughout Shelby County, including Sidney, Fort Loramie, and Anna.

Schedule:

  • Flexible shifts are available Monday through Sunday
  • Set your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Evaluate prior and current medical history, medical conditions, and medication history
  • Provide patient education on diagnosed conditions and prescribed medications
  • Identify gaps in care and recommend appropriate follow-up actions
  • Focus exclusively on assessmentno requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • At least 6 months of current NP experience preferred
  • MSN degree from an accredited school of nursing

You will receive $80$90 per evaluation, plus a mileage stipend and compensation for administrative time.
