Schedule:

Key Responsibilities:

• Conduct comprehensive, annual risk assessments in patient homes for Medicare Advantage members
• Review and document medical history, current conditions, and medication use
• Provide clear education to patients regarding their health conditions and medications
• Identify care gaps and recommend solutionsno treatment planning or prescribing required

Qualifications:

• Active Nurse Practitioner license in Ohio
• Current CPR certification (BLS)
• National Board Certification through AANP or ANCC
• Six months or more of recent NP experience preferred
• Master of Science in Nursing from an accredited institution
