Job Description

Experienced and compassionate Nurse Practitioners are sought to perform home health risk assessments for Medicare Advantage plan members throughout Miami County, with flexible per diem opportunities available. You will support patients living in locations such as Troy, Piqua, and Tipp City, providing critical healthcare insights without the responsibility of treatment plan creation or prescribing.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Ability to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct comprehensive home health risk assessments, averaging 45 minutes per visit, for Medicare Advantage members
  • Evaluate prior and current medical histories, chronic conditions, and medication usage
  • Offer patient education regarding medical conditions and medications
  • Identify gaps in care and recommend solutions for improved health outcomes
  • Visits may include annual health assessments or post-discharge check-ins

Qualifications:

  • Active Nurse Practitioner license in the state of Ohio
  • Board Certification as a Nurse Practitioner through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current CPR certification (BLS)
  • At least 6 months of recent Nurse Practitioner experience preferred

Successful candidates will receive $80$90 per completed evaluation, along with mileage stipends and compensation for related administrative tasks.
