This opportunity is ideal for a Nurse Practitioner seeking to make a significant impact on patient wellness within nursing home and assisted living environments. The program emphasizes comprehensive care for insurance plan members, with the goal of providing on-site treatments, reducing unnecessary hospital admissions, and ensuring smooth transitions to home settings. Health assessments are performed at least monthlyor more frequently if neededto promote better outcomes and greater stability for patients.
Schedule:
- MondayFriday, 8:00 AM to 5:00 PM
- No weekends or holidays required
Key Responsibilities:
- Develop and maintain problem lists and medical treatment plans for each resident
- Deliver primary, preventive, urgent, or acute care onsite at assigned facilities
- Identify and prioritize high-risk patients to enable more frequent clinical visits as necessary
- Create contingency plans for managing high-risk conditions
- Collaborate and communicate closely with each residents primary care physician
- Provide routine care, develop individualized care plans, and collaborate with facility staff, Directors of Nursing, and PCPs
- Conduct single visits for patients transitioning into facilities post-hospital discharge, ensuring safe transfers and effective discharge planning
- Maintain responsibility for a panel of members across multiple skilled nursing, assisted living, or related facilities; may occasionally travel between locations to assess acute conditions
Qualifications:
- Master of Science in Nursing (MSN)
- Active Nurse Practitioner license in New York
- BLS certification
Nurse Practitioners in this role can expect competitive hourly compensation, ranging from $75 to $100 per hour.
