Advance your career as a Nurse Practitioner working with patients in skilled nursing and assisted living environments on Staten Island, NY. This contract opportunity centers on comprehensive care for insurance members, emphasizing a proactive approach that prevents unnecessary hospital admissions and supports smooth transitions from facility to home.

Schedule:

• Monday to Friday, 8 AM 5 PM
• No weekends or holidays

Key Responsibilities:

• Conduct monthly (or as needed) health assessments for residents, focusing on improving in-place care and reducing hospitalizations.
• Formulate problem lists and individualized medical treatment plans for each patient.
• Provide primary, preventive, urgent, and acute care on-site in assigned facilities.
• Identify and prioritize high-risk patients, ensuring more frequent clinical follow-ups and detailed contingency planning.
• Collaborate and communicate with residents' primary care physicians and facility teamsincluding Directors of Nursing and PCPsto ensure continuity of care.
• Deliver hands-on bedside care, develop patient-centered care plans, and generate necessary medical orders.
• Support facility transitions for patients discharged from the hospital by assessing them and preparing safe, effective transition plans.
• Manage a panel of assigned patients across multiple skilled nursing or assisted living facilities, with primary assignments at one facility daily; flexibility to assess acute patients at other sites as needed.

Qualifications:

• Master of Science in Nursing (MSN)
• Active Nurse Practitioner (NP) license in New York
• Current Basic Life Support (BLS) certification
