Make a positive impact in your community by performing home health risk assessments as a Nurse Practitioner in Lawrence County, OH. This per diem opportunity allows you to use your clinical expertise to directly support individuals on Medicare Advantage plans, helping ensure they receive the best possible care in the comfort of their own homes.

Schedule:

• Flexible shifts available between Monday and Sunday
• Create your own schedule to fit your availability
• Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage health plan members
• Review prior and current medical history, evaluate medical conditions, and medication use
• Educate patients regarding existing medical conditions and proper medication management
• Identify gaps in care, recommending strategies for ongoing improvement
• Focus solely on assessmentsno development of treatment plans or medication prescribing necessary

Qualifications:

• Active Nurse Practitioner license in Ohio
• Valid CPR certification (BLS)
• Board Certification through AANP or ANCC
• 6 months of current Nurse Practitioner experience preferred
• MSN degree from an accredited nursing school
