Job Description

In this per diem role, youll conduct home health risk assessments for Medicare Advantage members across Sidney and the greater Shelby County area, supporting patient care in locations such as Fort Loramie and Anna.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Opportunity to create a personalized work schedule
  • Minimum weekly or bi-weekly hourly commitment as fits your availability

Responsibilities:

  • Complete annual risk assessments in patients homes, typically requiring about 45 minutes per visit
  • Evaluate each members current and past medical history, including medical conditions and medication usage
  • Educate individuals on their health conditions and prescribed medications
  • Identify care gaps and recommend next steps to ensure continuity and quality of care
  • No need to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Valid CPR certification (BLS)
  • Board certified through AANP or ANCC
  • MSN degree from an accredited program
  • Minimum of six months NP experience preferred
