Advance your career as a Nurse Practitioner by delivering vital home health risk assessments for Medicare Advantage members within Miami County. This per diem opportunity offers outstanding scheduling flexibility, empowering you to tailor your work hours to your life. Assessment visits last approximately 45 minutes and may entail annual health evaluations or post-discharge check-ins throughout areas such as Troy, Piqua, and Tipp City.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Design your own schedule to accommodate personal commitments
- Options for minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Perform comprehensive, in-home health risk assessments for Medicare Advantage members
- Review and evaluate medical history, current conditions, and medication use
- Offer clear patient education regarding health conditions and prescribed medications
- Identify gaps in healthcare and suggest actionable recommendations to resolve them
- No requirements to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certified through AANP or ANCC
- Minimum of 6 months recent NP experience is preferred
- Master of Science in Nursing (MSN) from an accredited institution
