A per diem Nurse Practitioner opportunity is available to conduct home health risk assessments within Highland County, Ohio. This role involves evaluating Medicare Advantage plan members through comprehensive home visits that typically last around 45 minutes. Visits may include annual health assessments or post-discharge evaluations to ensure patients receive the necessary education and support for managing their health conditions.
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage members
- Review and evaluate medical and medication histories
- Provide patient education regarding medical conditions and medication management
- Identify gaps in care and recommend solutions for improvement
- No treatment plan creation or medication prescribing required
Qualifications:
- Current Nurse Practitioner license in Ohio
- Board Certification through AANP or ANCC
- Minimum six months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
- Current CPR certification (BLS)
Schedule:
- Flexible shifts available Monday through Sunday

This role offers the chance to practice autonomously in community settings such as Hillsboro, Greenfield, and Leesburg within Highland County. Mileage stipend and administrative time are provided in addition to competitive pay per evaluation.
