Job Description

This per diem position offers a flexible schedule for an experienced Nurse Practitioner to conduct home health risk assessments within Huron County, OH, including Norwalk, Willard, and Bellevue. The role primarily involves assessing Medicare Advantage plan members through visits that typically last around 45 minutes. These visits may consist of annual health assessments or post-discharge evaluations.

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members.
  • Review and evaluate patients prior and current medical history, health conditions, and medication use.
  • Educate patients regarding their medical conditions and medications to promote understanding and compliance.
  • Identify any gaps in care and recommend appropriate resolutions.
  • No treatment plan creation or medication prescribing is required.

Qualifications:

  • Active Nurse Practitioner license in Ohio.
  • Current BLS (CPR) certification.
  • Board certification through AANP or ANCC.
  • Minimum of six months recent NP clinical experience preferred.
  • Master of Science in Nursing (MSN) from an accredited institution.

Schedule:

  • Flexible shifts Monday through Sunday.
  • Create your own schedule with a minimum weekly or bi-weekly commitment.
