Job Description

Provide vital healthcare assessments as a Nurse Practitioner conducting home health risk evaluations within Erie County, Ohio. This per diem role focuses on Medicare Advantage plan members, offering the flexibility to create your own schedule with shifts across Sandusky, Huron, and Vermilion.

Responsibilities:

  • Perform comprehensive annual home health risk assessments and post-discharge visits lasting approximately 45 minutes.
  • Review and evaluate patients medical histories, current conditions, and medication use.
  • Deliver patient education on medical conditions and medication management.
  • Identify gaps in care and offer recommendations to improve patient outcomes.
  • Note: No treatment plan development or medication prescribing is required.

Qualifications & Desired Experience:

  • Active Nurse Practitioner license in Ohio.
  • Board Certification through AANP or ANCC.
  • Current CPR certification (BLS).
  • Minimum of six months recent NP practice experience is preferred.
  • MSN degree from an accredited nursing program.

Schedule:

  • Flexible shifts available Monday through Sunday.
  • Ability to create a personalized work schedule for optimal work-life balance.
  • Minimum commitment: weekly or bi-weekly hours.
