Make a significant impact on patient care within the home setting by utilizing your skills as a Nurse Practitioner. In this per diem role, you will complete comprehensive home health risk assessments for Medicare Advantage plan members residing across Belmont County. With flexibility to create your own schedule, youll have the autonomy to balance personal and professional commitments while delivering exceptional, individualized healthcare.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Meet a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual health risk assessments and post-discharge visits in patient homes
- Evaluate medical history, current conditions, and medication use
- Provide patient education regarding chronic illness management, disease prevention, and medication adherence
- Identify care gaps and offer actionable recommendations to enhance patient outcomes
- Complete comprehensive documentation; no prescribing or treatment planning is required
Qualifications:
- Active Nurse Practitioner License in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC required
- MSN degree from an accredited school of nursing
- At least 6 months NP experience preferred
Compensation for this position includes $80-$90 per completed evaluation, along with a mileage stipend and administrative time, reflecting both your expertise and commitment to patient well-being.
