A healthcare opportunity is available for an experienced Nurse Practitioner (NP) specializing in long-term care settings. This role focuses on providing comprehensive care for patients in nursing homes and assisted living facilities. The goal is to manage health conditions in place, minimizing unnecessary hospitalizations and ensuring a smooth transition back to home care when appropriate.
Schedule:
- Monday through Friday
- 8 AM to 5 PM
- No weekends or holidays
Key Responsibilities:
- Develop individualized problem lists and medical treatment plans for residents.
- Deliver primary, preventive, urgent, and acute care directly on-site.
- Identify and stratify high-risk patients for more frequent clinical monitoring.
- Create contingency plans for managing high-risk health conditions.
- Collaborate closely with each residents primary care physician and facility staff.
- Conduct routine bedside patient care, develop care plans, create medical orders, and communicate with facility leadership including the Director of Nursing.
- Perform single visits for patients newly transitioned from hospital settings, ensuring safe, healthy transfers out of skilled nursing facilities.
- Manage assigned patient panels across multiple nursing or assisted living facilities and provide acute patient assessments as needed.
Desired Qualifications:
- Master of Science in Nursing (MSN) degree.
- Current New York Nurse Practitioner license.
- Basic Life Support (BLS) certification.
- DEA license.
- Prior experience in long-term care settings preferred.
