Advance patient care in the home health setting by performing meaningful risk assessments within Lawrence County, OH. This per diem opportunity offers Nurse Practitioners the chance to support Medicare Advantage plan members through in-home annual health assessments or post-discharge visits. The average visit lasts about 45 minutes, and daily assignments may take you to varied locations such as Ironton, South Point, or Proctorville, providing exposure to diverse clinical scenarios.

Schedule:

• Flexible shifts Monday through Sunday
• Customize your work schedule
• Commit to a minimum number of hours per week or every two weeks

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Review medical and medication histories, as well as previous and current conditions
• Provide targeted education regarding health conditions and prescribed medications
• Identify care gaps and offer actionable recommendations for improvement
• Note: This role does not include prescribing medications or designing treatment plans

Qualifications:

• Active Nurse Practitioner license in Ohio
• Current CPR certification (BLS)
• Board certification from AANP or ANCC
• At least 6 months of recent nurse practitioner experience is preferred
• MSN from an accredited nursing institution

As an added benefit, each evaluation is compensated at $80-$90, with an included mileage stipend and administrative time, supporting both your time and travel effort.
