Job Description
An opportunity is available for a Nurse Practitioner skilled in home health assessments to provide meaningful care to Medicare Advantage plan members throughout Scioto County. This per diem position empowers you to create your own schedule, offering maximum flexibility to fit your lifestyle.
Schedule:
- Flexible shifts offered between Monday and Sunday
- Self-schedulingdesign your own work hours
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete comprehensive annual or post-discharge home health risk assessments, each lasting approximately 45 minutes
- Evaluate patients medical history, current conditions, and medication regimens
- Deliver education to patients on medical issues and medications to support ongoing wellness
- Identify gaps in care or risk factors and furnish recommendations to enhance health outcomes
- No requirement to draft treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current Basic Life Support (BLS/CPR) certification
- Board Certification (AANP or ANCC)
- Preferred: Minimum of 6 months recent NP experience
- MSN degree from an accredited institution
The work will include traveling throughout communities such as Portsmouth, Porter Township, and Clay Township within Scioto County. For each completed evaluation, compensation ranges from $80-$90, along with a mileage stipend and pay for administrative time.
