Advance your Nurse Practitioner career with a unique per diem opportunity focused on home health risk assessments. In this role, you'll perform in-home evaluations for Medicare Advantage members throughout Scioto County, including Portsmouth, Porter Township, and Clay Township. Assessments typically last around 45 minutes and may include annual wellness visits or post-discharge follow-ups. You will provide valuable insights into members' health status, help close gaps in care, and offer educationall while enjoying autonomy and a self-directed schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own weekly or bi-weekly schedule
- Minimum commitment based on your availability
Key Responsibilities:
- Perform comprehensive annual health risk assessments in patients homes for Medicare Advantage plan members
- Review patients' medical history, current conditions, and medications
- Educate members on chronic conditions and medication management
- Identify care gaps, recommending solutions to improve outcomes
- Documentationno prescribing or creation of care plans required
Qualifications:
- Unrestricted Nurse Practitioner license in OH
- Board certified as an NP through AANP or ANCC
- MSN degree from an accredited nursing program
- Current BLS (CPR) certification
- Minimum of 6 months nurse practitioner experience preferred
