Job Description

Provide critical healthcare services as a Nurse Practitioner conducting home health risk assessments within Erie County, OH. This per diem role offers flexible scheduling from Monday through Sunday, allowing you to create your own shifts with a minimum weekly or bi-weekly hourly commitment. Your assessments will support Medicare Advantage plan members, focusing on annual health evaluations and post-discharge visits in locations including Sandusky, Huron, and Vermilion.

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Evaluate prior and current medical history, medical conditions, and medication history
  • Educate patients on their medical conditions and medications
  • Identify gaps in care and recommend actionable solutions
  • No treatment plan creation or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of 6 months NP experience preferred
  • MSN degree from an accredited nursing school
