Job Description

Make a meaningful impact on patient care by performing comprehensive home health risk assessments for Medicare Advantage plan members in Meigs County. This per diem position offers flexibility and autonomy, allowing you to build your own schedule with shifts available any day of the week. Assignments may take you to various locations throughout the county, including Pomeroy, Rutland Township, Salisbury Township, and Chester Township.

Schedule:

  • Flexible shifts available seven days a week
  • Set your own hours based on your availability
  • Minimum hourly commitment required weekly or bi-weekly

Key Responsibilities:

  • Perform annual home health risk assessments in patients homes
  • Review prior and current medical histories, conditions, and medications
  • Provide clear education to patients regarding their medical conditions and prescribed therapies
  • Identify care gaps and recommend follow-up actions to support optimal health
  • Focus exclusively on assessmentsno treatment planning, medication prescribing, or ongoing care management

Qualifications:

  • Active Nurse Practitioner (NP) license in Ohio
  • Current Basic Life Support (BLS/CPR) certification
  • Board certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least 6 months of current NP experience preferred
  • Strong communication and assessment skills, with a commitment to patient-centered care
